Special Information Form (with Form-2)
Fill in the Election Commission's আবেদনকারীর বিশেষ তথ্য ফরম, the extra information given with Form-2 in special areas, check it, and print it on A4.
The printed sheet follows the content of the Election Commission's আবেদনকারীর বিশেষ তথ্য ফরম (ফরম-২ এর অতিরিক্ত তথ্য) as published on its forms page (official PDF), in the same order and wording. Items 14–15 and the committee's signatures are for officials, so they print blank.
Who needs it: it is filled in together with Form-2 by people registering as voters in the EC's “special areas”. The EC's circular of 23 January 2025 lists all areas of 56 upazilas/thanas in Chattogram, Cox's Bazar, Rangamati, Khagrachhari and Bandarban districts. A special committee checks the NID numbers you give (item 8 relatives, item 6 grandfather) and decides (item 15). Ask your Upazila/Thana Election Office if you are unsure whether it applies to you.

FAQ
Which form is this?
“আবেদনকারীর বিশেষ তথ্য ফরম (ফরম-২ এর অতিরিক্ত তথ্য)”, the additional-information form added to the Voter List Rules 2012 by S.R.O. 11-আইন/2023 and published on the Election Commission's forms page (ecs.gov.bd). The printed sheet follows its two pages in the same order and wording. Design Dokan is not the Election Commission.
Who has to fill it in?
People registering as voters in the EC's special areas, together with Form-2. The EC's circular of 23 January 2025 declared all areas of 56 upazilas/thanas in Chattogram, Cox's Bazar, Rangamati, Khagrachhari and Bandarban districts special areas. Your Upazila/Thana Election Office can confirm whether it applies to you.
Why does it ask for three relatives' NID numbers?
The special committee checks those NID numbers online: per the EC's circular, the parents' names on a brother's or sister's NID should match yours, and an uncle's or aunt's father's name and address should match the grandfather details you give. Give relatives in the order the form asks (brother, sister, চাচা, ফুফু).
What should I attach?
If you live in your own house, the form asks for a photocopy of the land deed (item 9-ক). Bring the documents you bring for Form-2 as well. The EC's circular lists the papers it considers for each category, such as SSC certificate, QR-code online birth and citizenship certificates and parents' NIDs.
Where do I submit it?
With your Form-2, to the data collector or the Upazila/Thana Election Office. The special committee's decision is written in item 15.
