Voter Registration Form (Form-2)
Fill in the Election Commission's voter registration form, নিবন্ধন ফরম (ফরম-২), on screen, check the numbers, and print it on A4 ready to submit.
The printed sheet follows the content of the Election Commission's নিবন্ধন ফরম (ফরম-২) as published on its forms page (official PDF): same items, wording and boxes, with the registration slip (ফরম-৫) and receipt at the bottom. Items 34–46, the slip and the receipt are filled in by the identifier and election officials, so they print blank.
Prefer to apply online? The EC's NID portal has “নতুন নিবন্ধনের জন্য আবেদন” (apply for new registration); you print the form it makes and take it to the election office. This tool helps you prepare the paper form neatly.

How to use
- 1Fill in the fields in Bangla, the way the official Form-2 asks (your name in English goes in capitals). Leave anything you don't know blank.
- 2Tick “প্রযোজ্য নহে” where an item doesn't apply, and use “Same as present address” if your permanent address is the same.
- 3Press “Check the form”: it flags a wrong NID length, a 17-digit birth registration number that's short, a bad mobile number or a missing item.
- 4Press “Print / Save PDF”, print on A4 (scale 100%), then sign or give your thumbprint by hand and submit it with your documents.
FAQ
Is this the official Election Commission form?
The printed sheet reproduces the content of নিবন্ধন ফরম (ফরম-২) as published on the Election Commission's forms page (ecs.gov.bd → ভোটার নিবন্ধন সংক্রান্ত ফরম), in the same order and wording, with the registration slip (Form-5) and receipt. Design Dokan is not the Election Commission; compare with the official PDF linked above the form if you are unsure.
Where do I submit the form?
To the registration officer of your upazila or thana, through the Upazila/Thana Election Office (or the data collector during a voter-list update). Your photo, fingerprints and signature are taken at the registration centre (item 43).
Can I apply online instead?
Yes. The EC's NID portal, services.nidw.gov.bd, has “নতুন নিবন্ধনের জন্য আবেদন” (apply for new registration). You fill the form online, print it, and take it with your documents to the election office for biometrics. This tool is for preparing the paper form.
Which documents should I bring?
The EC's NID portal lists, as applicable: proof of age (SSC certificate, birth registration certificate, or passport/driving licence/TIN), proof you normally live in the area (utility bill, house-rent or holding-tax receipt), your parents' and husband's/wife's NID copies, and a citizenship certificate where it applies.
Is there a fee for the form?
The EC's NID portal says no fee is charged for these forms. You can also get a blank copy from the Upazila/Thana Election Office or download it from ecs.gov.bd.
Is my information sent anywhere?
No. The form is filled in and printed inside your browser. Nothing is uploaded. A draft is kept on your device only if you tick “Save a draft on this device”, and “Clear form” removes it.
