Voter List Inclusion Claim (Form-6)
Fill in the Election Commission's Form-6, নাম অন্তর্ভুক্তির দাবী, to claim inclusion in the draft voter list, check it, and print it on A4 ready to submit.
The printed sheet follows the content of the Election Commission's ফরম-৬ “নাম অন্তর্ভুক্তির দাবী” (claim to include a name in the voter list) as published on its forms page (official PDF), with Part 1 and the notice (Part 2) in the same order and wording. Everything the revising authority's office fills in prints blank.
When to use it: Form-6 is the claim form used when the Election Commission publishes a draft voter list and invites claims, objections and corrections. Submit it to the revising authority (সংশোধনকারী কর্তৃপক্ষ) named in that notice, within the dates given. Item 7 says a filled-in Form-2, signed by the identifier, must be attached. Outside a claims period, registering as a new voter is done with Form-2 itself: the older Form-11 (“ভোটার তালিকায় নতুন নাম অন্তর্ভুক্তির আবেদন”) was abolished by S.R.O. 11-আইন/2023.

How to use
- 1Fill in your name, your father's/husband's and mother's names and their voter or NID numbers, and why you could not be registered.
- 2Choose whether you submit the claim yourself or through a representative (the other wording is struck out on the printout), and add your full address for the notice.
- 3Press “Check the form”, then “Print / Save PDF” and print both pages on A4.
- 4Sign or give your thumbprint, attach a filled-in Form-2 signed by the identifier, and submit it to the revising authority within the claims period.
FAQ
Which form is this?
ফরম-৬ [বিধি ১৬ ও ১৮(৪)], “নাম অন্তর্ভুক্তির দাবী”: the claim to include a name in the voter list, as published on the Election Commission's forms page (ecs.gov.bd). The printed sheet follows its Part 1 and Part 2 (notice) in the same order and wording. Design Dokan is not the Election Commission.
When can I use Form-6?
When the Election Commission publishes a draft voter list and invites claims, objections and corrections. Its notice names the revising authorities (সংশোধনকারী কর্তৃপক্ষ) and the last date. Check the notice from your Upazila/Thana Election Office or ecs.gov.bd for the current dates.
What about Form-11, “ভোটার তালিকায় নতুন নাম অন্তর্ভুক্তির আবেদন”?
Form-11 was abolished by S.R.O. 11-আইন/2023 (Bangladesh Gazette, 12 February 2023). Rule 26(2) now says a person claiming inclusion fills in Form-2 and submits it to the registration officer. Outside a claims period, use our Voter Registration Form (Form-2) tool.
What must I attach?
Item 7 of the form says a filled-in Form-2, signed by the identifier (শনাক্তকারী), must be attached. Bring the documents you would bring for Form-2 too, such as proof of age and of your address.
Can someone submit the claim for me?
Yes. Item 8 lets you authorise a representative: give their name and voter or NID number, and they sign item 8(গ). Otherwise choose “আমি নিজে এই দাবী পেশ করিলাম”.
Is my information sent anywhere?
No. Everything is filled in and printed inside your browser; nothing is uploaded. A draft is kept on your device only if you tick “Save a draft on this device”, and “Clear form” removes it.
